Hospital Patient Portal

On November 2, 2021, Crozer Health implemented a new electronic medical record system that includes a new consolidated patient portal, called Health+Link, for inpatient and outpatient care. This technology enables us to have faster and more convenient access to your records wherever you go within the Crozer Health system and provide you with more efficient and effective care. 

The CKHospital patient portal was discontinued on November 1, 2021. Patients can access their care records through the portal until the end of November 2021. You can also request copies of your records from your doctor at any time.

After November 2, 2021 all care records (inpatient and outpatient) can be found in the new Health+Life portal. We encourage you to sign up for Health+Link https://www.crozerhealth.org/portal-login/.

View, Download and Share - Safely and Securely

The Crozer Health Hospital Patient Portal is a user-friendly website that gives you the information and resources you need to better manage and coordinate your healthcare. The portal is secure and private, easy-to-use and enables you to view and download information about your encounters at any Crozer Health hospital.

With the Hospital Patient Portal, you can:

  • View vital information such as lab test results, X-ray and MRI reports and findings from other diagnostic procedures.
  • Download and save information about allergies, medications, conditions, immunizations and more.
  • View a list of all your healthcare encounters with Crozer Health hospitals.
  • Securely and safely share your health information with providers, caregivers and family.

How to Access Your Account

SIGN IN TO THE PORTAL HERE

  • Enter the user ID and password that you may have received from hospital staff during your inpatient stay.

  • A hospital patient portal is an online tool that enables patients to access and view the records of care from their hospital encounter. From the portal, patients can download, save, print and share this information.

  • Crozer Health is happy to provide access to our Hospital Patient Portal at no charge to you.

    • Safe, secure access to your health information from Crozer Health:
      • Demographic information
      • Laboratory results
      • Radiology results
      • Medications
      • Allergies
      • Medical conditions
      • Discharge Instructions
    • Health resources such as events, health calculators and education
    • Ability to share your health records with your doctors, caregivers and family.
    • If you added an email address to your account, visit www.ckhospitalportal.org and click "Forgot your password?" on the login screen. Enter your email address to have your password sent to you.
    • If you did not add an email address to your account, submit a support ticket from the login screen at www.ckhospitalportal.org. Click on the link for "Having trouble logging in?" Be sure to include your contact information and someone will follow up with you.
    • Select "My Setup" from the left menu, then click "Password Change."
    • You will be asked to verify your UserID and current password, then to enter your new password two times.

  • Login to www.ckhospitalportal.org and select "My Health Record," then "Health Summary" from the left menu. You can then select from the following:

    • Lab/Outpatient Radiology Results: View, trend or graph your laboratory results.
    • Reports: View your transcribed reports such as discharge summaries, progress notes, emergency department reports, etc.
    • Health Summary: View or download your health summary documents in a PDF format. The Health Summary will include all of the following care records and results provided during your hospital stay: laboratory and radiology results, medications, allergies, medical conditions and discharge instructions.
    • Visits: View recent visits.

    For instructions on how to use each of the screens, such as how to download your documents, please reference the tutorials that appear at the top of the screen.

  • Yes! All information within the Crozer Health Hospital Patient Portal is secure, meeting the highest healthcare standards. Only you, your healthcare providers and those you provide your information to will see your information.

  • Log on to www.ckhospitalportal.org and click "Send a secure support ticket here."

  • The Crozer Health Hospital Patient Portal "times out" after a period of inactivity to protect your information from unauthorized access. Log in again to see your information securely.

  • Yes, you can bookmark the Crozer Health Hospital Patient Portal using your Internet browser's bookmark function.

    • For Windows: Right click on the mouse and a menu pops up. Then click on "create a shortcut." A window will pop up that says, "a shortcut to the current page will be placed on your desktop." Click on "OK."
    • For Mac: Go to the webpage and right click: "create shortcut."
