Medical Imaging Patient Portal
Crozer Health's Medical Imaging Services offers patients Health Companion, a secure portal for medical imaging patients to access their healthcare information. You can view your clinical summary that may include any known problems, current medications, allergies and more.
Crozer Health Medical Imaging services patients must register with Health Companion within 30 days of receiving care by following these instructions.
How to Register
Use this link if you have been a medical imaging patient at Crozer-Chester Medical Center, Taylor Hospital, Springfield Hospital, Media Medical Imaging, and/or Brinton Lake Imaging.
Use this link if you have been a medical imaging patient at Delaware County Memorial Hospital (DCMH) and/or Broomall Imaging.
- Verify your identity by entering a PIN that was provided to you by Crozer Health's Medical Imaging department.
If you were a patient at Crozer/Taylor/Springfield or Media Medical Imaging, click on the first link above.
If you were a patient at DCMH, click on the second link above.
- Enter your PIN, first name, last name, date of birth and verification code (which appears above the 'Enter the Verification Code field) and press search. A box should appear that will allow you to mark that you have read and agree to the Terms and Condition. Once you have marked this box, you will be prompted with the following questions:
Are you the individual identified above? Please mark "Yes" if that statement is true.
Are you already registered as a user in Health Companion? Please select "No" and you will be prompted to create your username, password and select security questions/answers.
- After you enter this information, please click "Submit." You will be prompted to enter your newly-created username and password and will then be directed to your Health Companion account and will have access to your health information.
Note: The verification code will change each time the web page is refreshed, and will also disappear from the field that you entered it into once you click "Search." However, once you see the Terms and Conditions box appear, you can ignore the verification code field and everything above it as the site was able to successfully find your information.
To Reset Username/Password
To reset your username/password, go to http://www.healthcompanion.com and click "Forgot Username." Health Companion will email you your username. You can then click "Forgot Password" and you will be prompted to answer two of the security questions you set up during the initial registration process. Once you successfully answer these questions, you will be able to reset your password.
