Crozer Health’s computer systems are now back up and running as normal and the health system is continuing to provide safe, quality care to patients following a data security incident that disrupted our operations. Work to input paper patient records used by our caregivers while our systems were down into our electronic medical record (EMR) system is ongoing.  We appreciate the resilience shown by our physicians and employees during this disruption and we thank our patients for their patience and continued loyalty to our facilities. Delivering safe, quality care is our most important priority.

Small group instruction and overview of diabetes; causes, symptoms, recommended action for hyperglycemia and hypoglycemia; benefits of exercise; overview of medications and foot care.

  • Small group instruction and overview of diabetes; causes, symptoms, recommended action for hyperglycemia and hypoglycemia; benefits of exercise; overview of medications and foot care. Review American Diabetes Association (ADA) guidelines: blood sugar goals, Hemoglobin A1c, and Standards of Care.

    Program participants may be required to have an insurance referral and/or a signed doctor note. This information can be faxed to 610-328-8922. Please check with your insurance carrier regarding your coverage.

    All participants are encouraged to meet at The Center for Diabetes prior to the start of class and a member of the team will guide the class to the conference room.
