Gwen A. Smith Award for Community Service
The Gwen Smith Community Service Award was created to honor Crozer Health employees, like Gwen, who make significant contributions to our community through their time, talent, actions, and dedication. The award recognizes employees’ unyielding dedication to community service, and to improving the lives of others. It is presented to a deserving colleague each April in conjunction with National Volunteer Week.
Call for Nominations
Do you know a Crozer Health team member who serves as a role model for compassion and service and a passion for helping others in our community, who has done amazing things over the past year to help others? If so, please nominate them for the Gwen Smith Community Service Award. Nominations for the 2022 award are due by Monday, February 14.
Nominees for this award will have consistently demonstrated qualities of compassion and service to one or more organizations serving our community. This can include a single or variety of volunteer activities throughout the year. Criteria include:
This person makes significant contributions to building a sense of unity and purpose at work and in our community. The nominee should demonstrate the ability to inspire and encourage others through their actions.
This person is committed to improving our community through volunteer programs, social groups, outreach centers, sports programs, after-school groups, or other charitable work. The nominee should have contributed to at least one community program or initiative with measurable outcomes.
This person demonstrates our core values to inspire and encourage others through their actions. The nominee should have examples of instances during which they have mentored individuals in a positive way.
This person influences others to effect positive changes. They should have examples of their impact as a change agent.
How to Nominate
Nominations may be submitted by anyone in our community, including employees and members of the non-profits where nominees serve. Self-nomination is also encouraged. Please use the form below.

Crozer Health will recognize a maximum of five finalists with one overall winner selected by the award committee as the award recipient. We will recognize the finalists at an award ceremony on April 14, 2022..
About Gwen Smith
Gwendolyn A. Smith, R.N., MBA, M.S., MSM is a treasured member of the Crozer Health family. Having served Crozer Health for 41 years before her official retirement in June 2021, she is still graciously giving much of her time to help the health system – and her community – in many ways.
Since joining Crozer Health as a nurse in 1980, Smith’s many roles included Vice President/Patient Experience, President of Springfield Hospital, Vice President of Community Health, and Director of Nursing. During the COVID-19 pandemic, she came out of semi-retirement to hold community vaccination clinics in Delaware County, and she personally administered more than 20,000 vaccines.
Smith’s passion for people stems from two principles from Dr. Martin Luther King, Jr., who she was privileged to meet as a young girl while he was studying at the Crozer Theological Seminary in Chester: “I’m not all I can be until you are all you can be.” and “Our destinies are tied together; none of us can make it alone.”
Gwen has been described as a leader and mentor who is selfless and dedicated to patients and the community. We can think of no better way to thank her for all she’s done than to create an award in her honor.
