Our Medical Home
The Center for Family Health(CFH) is the main clinical site for outpatient teaching. The physical space contains 15 patient exam rooms, one procedure room, a conference room with AV equipment, and resident and attending offices. Working in the CFH feels like “coming home” to residents, who often rotate on inpatient or outpatient specialty rotations on other days of the week. It is a place to hone the art of family medicine while caring for continuity patients in a supportive environment.
The CFH office is staffed by a Practice Manager, medical records clerks, front desk receptionists, medical assistants, and a nurse case manager. The residents and faculty are divided into clinical teams that work together to provide care to a panel of patients. This team concept involves other office staff and contributes to a collaborative approach to patient care. Residents are able to access their patient’s office charts remotely, via a link to the office electronic medical record. This facilitates quality care after hours and helps to reconcile medications and problem lists when patients are admitted to the hospital.
In 2020 our program was certified in Lifestyle Medicine through the American College of Lifestyle Medicine and provides an active LM learning environment for all residents. Dr. Deb Malloy, Director of Lifestyle Medicine, is a member of the American College of Lifestyle Medicine and certified by the American Board of Lifestyle Medicine. Residents who complete the curriculum can get an additional board certification in lifestyle medicine upon graduation.
In 2018 our Center was chosen as one of the "Top 3" Residency Clinics in the country by the AAMC and the UCSF, who also highlighted the CFH as a "High Performing Primary Care Residency Clinic" in 2016.
In June 2009, the office was awarded the coveted National Committee for Quality Assurance (NCQA) Level 3 designation as a Patient-Centered Medical Home. This quality award was based upon the residency program’s work with patient registries, chronic disease management, patient self-management support, and multi-cultural patient education activities. The CFH participates in quality initiatives related to cancer screening, flu vaccination, and pediatric care.
The Center for Family Health integrates the full scope of family medicine and health services for all ages as well as screening and education programs. As a primary care referral site for the Crozer Health Medical Group, the CFH is a busy office, seeing over 17,000 visits in 2021. Services include general medical care, wellness and preventive services, obstetrical and prenatal care, newborn and pediatric care, weight management, treatment of work and sports injuries and minor emergencies, lab and other tests, nutritional counseling, and special services for the elderly.
How to Apply and Interview
All applications to the Crozer Health Family Medicine Residency Program must come through the Electronic Residency Application Service (ERAS). The deadline for receiving applications is December 31.
