Medicare Scores Favorable for Crozer Health Home Health
Crozer Health Home Health was notified by the Centers for Medicare and Medicaid Services (CMS) that its most recent score for quality of care and patient experience was four stars out of five, which is above the average of home health agencies measured across the country.
Among Crozer Health Home Health’s highest scores were management of oral medications, bed transferring and improvement in dyspnea (shortness of breath).
“With a trend in health care now favoring outpatient and care at home, making sure our home health services provide excellent experiences and outcomes is a priority,” said Crozer Health CEO, Sharif Omar. “We’ve developed a full continuum of care that takes place in our patients’ homes, knowing they need our expertise there as much as they need it in our hospitals.”
In addition to skilled nursing and rehabilitative therapies, Crozer Health Home Health provides medical social services, pain management, wound care and ostomy assistance. The agency’s Mother-Baby Connection program provides support for recently discharged mothers and their infants, including lactation-certified nurses. The Support Care program is specialized for more complex home care needs such as pain or chemotherapy symptom management. And in-home hospice lends support for terminally ill people and their loved ones, encouraging the continuation of a meaningful life in the presence of a life-limiting illness.
