Crozer Health's Home Health Services provide care in the comfort of patients' homes. These patients may be newly discharged from the hospital, referred directly by physicians' offices.

Through the use of a home healthcare program, patients and their families are assured that a highly skilled professional staff provides care for individuals or those making the transition from hospital to home.
Home healthcare may shorten or eliminate the need for an inpatient hospital stay or may help the patient delay or avoid a nursing home placement.
Access 24 Hours a Day, Seven Days a Week
Home health patients have access to a nurse by phone 24 hours a day, seven days a week. In addition, our Home Health professionals carry laptop computers to allow them to keep your health records up-to-date and communicate electronically with other members of the Home Health team.

Coordinated Team Approach to Care
Healthcare and supportive skilled services are provided through a team approach that incorporates Crozer Health's quality care. The Home Health team, a group of highly skilled professionals, works in cooperation with patients' physicians to formulate an individualized plan for care. Our registered nurses and therapists coordinate your home care services under your physician's guidance. Even though we keep your physician up-to-date on your home care progress, you will continue to make regular office visits.

Mother-Baby Connection
Our special Mother-Baby Connection provides support for recently discharged mothers and infants, lactation-certified nurses for mothers who choose breastfeeding and phototherapy at home so infants can go home when their mothers do.
Supportive Care
Crozer Health's Support Care Program is a specialized home health service designed for individuals with more complex home care needs. You might be dealing with troublesome symptoms such as pain, fatigue or you might be juggling a busy schedule of chemotherapy, radiation therapy or periodic transfusions.
Your physician may recommend you for the Supportive Care Program because he or she feels it will best meet your home care needs. Our supportive care nurses specialize in pain management and symptom control for our Supportive Care and Hospice Program patients.
All services are done under the direction of your doctor and coordinated by an assigned primary nurse. Patients have access to emergency assistance by a registered nurse - 24-hours a day, 7 days a week.

How to Receive Services
A patient's family member, friend, physician or other healthcare professional may request home care services. Acceptance is based on approval by the patient's physician and other medical criteria. The Crozer Health Home Health staff will gladly work with anyone who is interested in home care services to evaluate patient needs and eligibility and formulate an individualized plan of care.
Payment
Our home care intake specialists will make all of the necessary arrangements and perform a full evaluation of your insurance coverage prior to your first home care visit.
