Having an Annual Wellness Visit helps to prevent disease and disability while creating a personalized plan with your provider to stay well!
The Annual Wellness Visit is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. This plan may help prevent illness based on your current health and risk factors.

Your visit can also include:
- Health risk assessment
- Medical and family history review
- Cognitive impairment screening
- Vital Signs
- Preventive care screening history and services
- Review risk factors & treatment options
- Advanced care planning (planning for future healthcare)
What does Medicare cover? What do I pay?
The better you take care of your car, the better it will work. The same goes for your body. Visit is covered 100% every 12 months with Medicare Part B!
However, if your doctor performs additional tests or services during this same visit or addresses other medical concerns (like a sore knee or other medical conditions) then you may have a deductible or copay.
With some preventive care, you can avoid an expensive repair!
What do I bring to my annual wellness appointment?
Gather the following items and bring them with you to your appointment:
- Medications
- Names of all your doctors
- Pharmacy names and locations
- Home health agency name
- Medical equipment company name
- Recent immunizations
- Health Risk Assessment Form (if your doctor’s office asked you to complete one)
- Copy of your living will (if available and not already provided to the office)
