In addition, we are faced with increasing health challenges that we didn't need to cope with when we were younger.

Crozer Health ’s Senior Health Services can help you cope with the many health and lifestyle changes you face. Crozer Health's dedicated professionals, programs and services form a comprehensive system of care for our community’s seniors.

Crozer Health offers the following community programs and services specifically geared towards you.

Senior Support Line

The Crozer Health Senior Support Line is a toll-free number that connects and refers seniors, their families and physicians to a host of local, state and national agencies whose mission is to support and assist seniors.

Some areas where we provide assistance include:

• Adult day services
• Nursing home information
• Nursing home referrals
• Health insurance eligibility determination
• Prescription eligibility determination
• Home-delivered meals
• Transportation

Dining at Dusk & Physician Lecture Series

Crozer Health offers senior-focused community health education through presentations that offer tips on how to partner with your health providers in managing your own health.

Dining at Dusk: A dinner program held at Springfield Hospital. Attendees participate in senior-focused presentations generally offered by Crozer Health physicians and clinical staff.

Physician Lecture Series: A free lecture series offering older adults an opportunity to learn important health-related information.

Geriatric Resource Center at Springfield Hospital

The Geriatric Resource Center gives older adults, children of aging parents and caregivers access to educational information on aging-related health topics. You will also find helpful information and applications for local, state and national senior assistance services and programs.

The information provided at the resource center includes topics such as keys to finding the right retirement or long-term care community in Delaware County and surrounding counties, tips on long-distance caregiving, how to choose a doctor, and mourning the death of a spouse. The center also has information on preparing for surgery, diseases like diabetes in the elderly, and dealing with depression. The resource center also has applications for PACE and PACENET as well as the Pennsylvania Low Income Energy Assistance Program.

Volunteers staff the Center that features a library of pamphlets, books and other materials.
